We implemented the following process for the Roosevelt Park pilot neighborhood.

High levels of health and economic disparities are present

moving forward, we will utilize the following criteria to select neighborhoods:

  • Health equity drivers, including presence of chronic disease, life expectancy, racial disparities, and social vulnerability
  • Local backbone organization
  • Local buy-in

Roosevelt Park asset mapping:

Includes a brief history of the neighborhood, a description of community assets, and demographic information. 

Roosevelt Park community assessments:

Includes focus groups and a door-to-door survey
