We implemented the following process for the Roosevelt Park pilot neighborhood.

Health equity drivers, including presence of chronic disease, life expectancy, racial disparities, and social vulnerability

Local backbone organization
Local buy-in

Roosevelt Park asset mapping:

Includes a brief history of the neighborhood, a description of community assets, and demographic information.

Roosevelt Park community assessments:

Includes focus groups and a door-to-door survey
